Staying at Home
If home is where you want to be, it’s essential that you and your carers have as much support as possible. Caring can be hard work, both physically and emotionally. It’s not always easy to ask for help, and many people find it impossible. However, it’s important that your carers save enough energy to do the things that only they can do, such as spending time talking to you and supporting you. They can pass on some jobs to other people, where possible.
The Care Plan
If you or someone you know does not have a care plan in place however you feel this would be beneficial, please discuss this with the Practice team who will be able to support you with the request.
Community Nurses
District Nurses carry out many roles for housebound patients, including:
- General nursing procedures and investigations
- Post-surgery care
- Assessment of patients’ needs
- Care for terminally ill patients
- Loaning of equipment
- Looking after wounds and leg ulcers
- Educating patients about self-care
Referrals can be made by a Doctor or nurse at the practice, patients themselves or by carers and family members.
Integrated Community Ageing Team (iCAT)
The ICAT is a multidisciplinary team (MDT) of Pharmacists, Pharmacy Technicians, Social Prescriber, Care co-ordinators and GP specialise in supporting patients living with moderate or severe frailty. They complete a Comprehensive Geriatric Assessments (CGA) for patients who are registered with an Eden GP and liaise directly with the GP team. This is usually carried out in the patient’s own home. They can support with medication, arranging care, liaising with community occupational therapist and nursing, referring on for memory assessment, carer support and overall, they support the patient in what matters to them.
The team are part of the primary care network initiative developed in 2020 to pursue our vision of wrapping care around our most vulnerable patients.
